This on-line form is coming soon. These grants are available to any Lyme patient or their family member who may be a potential Lyme patient. Information on income must be provided to demonstrate financial need. Anyone may qualify from anywhere in the world. Grants are distributed monthly and are paid to qualifying laboratories on behalf of the patient grantee. Deadlines for applications are the first of any given month. Applications received after that time period are considered during the following calendar month. All grant decisions are final and are subject to fund availability.
